From a plastic surgeonās perspective, bone loss plays a critical yet often underappreciated role in facial aging. As the facial skeleton resorbs over timeāparticularly in the zygoma, temples, maxilla, mandible, and orbital rimāsoft tissues lose structural support, leading to sagging, hollowness, and changes in facial proportions. Traditional approaches that rely solely on lifting and volumization may provide temporary improvement but fail to address the foundational issue of skeletal support loss. Instead, strategic use of injectable fillers, in addition to surgical approaches, can help restore bony contour and projection, providing a more natural and long-lasting rejuvenation. A balanced approachācombining skeletal augmentation with careful repositioning of soft tissuesāis essential for truly restoring youthful facial architecture rather than simply pulling or overfilling, which can lead to unnatural results. This is why I balance my practice with both surgical and non-surgical procedures, to allow the most precise, beautiful, and bespoke outcomes for my patients.
