🌱⚠️🫀 Bundle branch blocks are conduction disturbances of the fast electrical pathways which distribute action potentials throughout the ventricles. Bundle branches ensure that ventricular depolarization happens in an organized manner, creating an efficient contraction of ventricular muscle towards ventricular outflow tracts that lead to pulmonary and central circulation. These fast pathways conduct the action potential quicker than the cell to cell transmission of non-specialized myocardium proper. This fast conduction is what results in the normal QRS duration of less than 120ms. When these bundle branches are disrupted the ventricles rely on slower transmission of cells not specialized to conduct signals quickly. This slower propagation results in a wider QRS complex (> 120ms). Anatomically, there are two bundle branches, right and left. Damage to either changes the vector of electrical activity, changing the morphology of the QRS complex in a characteristic way that allows determining which bundle branch is blocked. There are more criteria than just what's visible in V1, but the shape of the QRS complex in V1 is a helpful pointer to which -the right or left- bundle branch is blocked. In this video we see a simple example of how to differentiate left and right bundle branch blocks using the terminal deflection of the QRS complex as seen in V1. A negative terminal deflection (return to baseline) in V1 points to a left bundle branch block. A positive terminal deflection (return to baseline) in V1 indicates a right bundle branch block. 😵🫀🤔 Here we have a recording of a witnessed ventricular fibrillation cardiac arrest that received immediate bystander CPR followed by ACLS care by EMS. During resuscitation efforts, the pt shows signs of consciousness such as: eye movement with fixation on rescuers, extremity movement, and hand grabbing of equipment. These signs disappear with the cessation of chest compressions, and return with their resumption after rhythm checks. This phenomenon is known as CPR Induced Consciousness (CPRIC). While a relatively newly described phenomenon in the literature, many EMS services have developed protocols for sedation and analgesia during resuscitation to address it. While these efforts should never take away from standard ACLS care, if providers are able, they should attempt to address the CPRIC with sedation and analgesia.
