#RNlife: A glimpse into the fast-paced world of registered nurses. Showcasing resilience, patient care, teamwork, long shifts, late nights, and the emotional rollercoaster of a healthcare hero's journey.
When drawing blood, occasionally the vacuum will suck the vein wall against the bevel of the needle, abruptly stopping blood flow, or causing a sort of “vibration”. . If this happens, try pointing your needle downward or simply pressing straight down with your needle to release it from the vein wall. This should restore blood flow. . If blood flow is still not restored, make sure your hands aren’t obstructing flow from distal veins, then try a new tube ( as the current tube may not have had enough vacuum). . Phlebotomy For Nurses (plus the IV Video Course, ultrasound IV insertion, AND Pediatric Venipuncture) are all 30% off and come with CEUs!!! This is called “chasing the bevel”, which we explain thoroughly in the Ultrasound IV Insertion Video Course. All my video courses are 30% off and come with continuing education! (Which often makes them eligible for reimbursement). Click the link in my profile or head to www.TheIVGuy.com/courses to learn more! . “Chasing the bevel” is done when the needle bevel enters the vein. As soon as you see it within the lumen, lower your angle of insertion, slide your probe away (proximal on the patient) a few millimeters, then advance your needle until you see it on the screen again, then slide the probe away, then advance again, all while keeping the bevel centered within the lumen of the vein. . This allows you to make sure there’s adequate catheter within the lumen, while visually confirming the needle has not entered the opposite wall. You can then set your probe down and thread your catheter. . #nurse #nursing #rn #rnlife #nurselife #instanursing #nurseproblems #nursingschool #nursingproblems #nursingschoolproblems #nursesofinstagram #nurses #studentnurse #nursingstudent #registerednurse #lvn #instanurse #nclex #nclexrn #nclexprep #nursingclinicals #clinicals #nursingschoolclinicals #nursingstudents #nursinglife #nclexreview #nclexpreparation #nclexquestions #peds #nicu

This clip demonstrates two techniques I talk about frequently. . Since the catheter sits behind the bevel of the needle, it’s still in the wall of the vein when you first see flash. Lowering your angle and advancing 2-6mm will ensure the catheter is all the way in the lumen of the vein, and will limit catheter contact with the vein wall while threading. . Tamponade can help stop blood from flowing out of the catheter after you remove the needle (if you aren’t using “blood control” catheters). . Once you thread the catheter, remove your tourniquet, then as you remove the needle, put pressure down on the vein just above the catheter end. This should occlude the catheter and limit leakage as you attach your extension tubing. . You can also place a stack of sterile gauze under the catheter hub before removing the needle. . . *this model is made of ballistic gelatin and is merely a representation of a vein. The gelatin is softer than human tissue and cannot mimic veins perfectly.* . The IV Video Course is 30% off (same with Ultrasound IV Insertion, Phlebotomy for Nurses, AND Pediatric Venipuncture). All my video courses are 30% off with larger discounts on “bundles”, AND they all come with continuing education! If you want to learn how to insert and maintain peripheral IVs and/or draw blood, just click the link in my profile or head to www.theivguy.com/courses 😊 . IV removal isn’t complicated, but I see it done wrong way too often. If done incorrectly it hurts (ask me how I know 😉), and can cause further damage to the vein. . When discontinuing an IV, first remove all tape and dressings. If the patient has fragile skin or a statlock was used, alcohol will help prevent skin tears. Rub the edges with a stack of alcohol pads, then continue rubbing under the dressings as you remove them. . When all the tape/dressings are removed from the skin, fold a stack of gauze twice and hold it over the insertion site. DO NOT PUT PRESSURE DOWN UNTIL THE CATHETER IS COMPLETELY REMOVED. . Pull the catheter out slowly (faster is not less painful), and as soon as it exits the skin, put pressure down with your gauze. . Inspect the catheter to ensure that it is intact. If it isn’t, or you suspect that a piece of catheter was left in the patient, contact the physician. . Place a piece of tape crosswise over the gauze, stretching each side slightly before securing it to the skin. This will create a sort of pressure dressing and prevent bruising and bleeding from the site. . #theivguy #ivstart #nurse #nursing #rn #rnlife #nurselife #instanursing #nurseproblems #nursingschool #nursingproblems #nursingschoolproblems #nursesofinstagram #nurses #studentnurse #nursingstudent #registerednurse #lvn #instanurse #nclex #nclexrn #nclexprep #nursingclinicals #clinicals #nursingschoolclinicals #nursingstudents

How to tie a tourniquet! . This is a simple one, but can make all the difference. Too often I see nurses struggling to palpate a vein and find that their tourniquet is barely sitting on top of the patient’s skin. This’ll get you nowhere in a hurry. . The point of a tourniquet is to cut off venous return without effecting arterial supply. It needs to be tight but not TOO tight. My advice is to make it hurt (just a little), and then it’s probably just right. . Place your tourniquet flat behind the patient’s arm, 4-6” above where you want to palpate. Stretch one side tight with your dominant hand, then grab it with your nondominant hand and hold it in place while you stretch the other side tight. Then take that end and tuck it from the top down under the other end. This holds them in place while keeping the tourniquet ends out of your venipuncture site. . To remove the tourniquet, pull straight up on BOTH ends. If you pull just one, the other end can end up smacking you or the patient in the face (ask me how I know 😉). . If you want to learn everything you need to know to start IVs, check out the IV Video Course! It’s 30% off (as are all my courses)! Here's another example of a tip I give a lot. The "Flash-pause-lower-advance-thread" technique. When you first get flash, the bevel of the needle is in the lumen of the vein. If you thread the catheter from here, you'll likely blow the vein as the end of the catheter may still be in the wall of the vein (it sits BEHIND the bevel of the needle). When you see flash, pause, lower your angle of insertion, advance another 2-6mm depending on gauge, THEN thread the catheter. This will make absolutely sure the catheter is within the lumen of the vein before threading and limit your chances of blowing it. . This clip is straight out of the IV Video Course, which is currently 30% off and comes with continuing education! The fact that it comes with CEUs from an ANCC accredited provider means it's likely eligible for reimbursement from your employer, making the course FREE! The original IV video course was updated last year and has new content added to meet the revised INS Standards of Practice. This kind of update will happen periodically when new standards are released or new best practices are identified. If you buy any of my courses, you’ll have lifetime access to any updated material I add in the future 😊 . There’s a whole section on pain management including topical and injectable lidocaine, vein marking techniques, and a few new full-length demonstrations! The brand new ultrasound course has a large section on pain management as well and focuses on injectable lidocaine. Check it out by clicking the link in my profile or head to www.theivguy.com/courses (they’re all 30% off and come with CEUs!!!) . Pain associated with venipuncture is often overlooked in the adult population, but is still extremely important and we need to do what we can to reduce pain as much as possible, whenever possible. . Topical anesthetic can be an easy, effective method of doing so, though it does take some extra time. . Palpate for a couple appropriate vessels, apply topical anesthetic and cover with a transparent dressing for 30-45 minutes. Go do some other tasks, then come back and perform your venipuncture.
