Acne can leave a variety of different types of scars. My patient here has a combination of ice pick and boxcar scars. Here you can watch me diagnose these scars and prepare a custom treatment plan for them. Let’s breakdown the types of scars: Ice pick scars are those deep, narrow pitted scars that can resemble, well, ice pick holes! They occur when there's significant inflammation deep within the skin. Boxcar scars have sharper edges and a wider, more rectangular shape, almost like little boxcars indented in the skin. These form when there's significant collagen loss in the deeper layers of the skin. Now, let's talk about the redness you might be seeing. Sometimes, post-inflammatory hyperpigmentation (PIH) can linger after acne has cleared. This redness is caused by an overproduction of melanin, the pigment in our skin. It can be mistaken for active acne which we were able to normalize for our patient with our active acne laser treatment. Want to see more of this patient’s journey - comment below. Ready to book a consultation? Comment "Qazi" below to set up your consultation and let’s build a plan to transform your skin. Got questions about this process, your skincare or treatment options? The second phase of this patient’s acne scar treatment is all about refining her results and targeting the deeper, more stubborn scars that require extra attention. In this video, you’ll see me walking her through the plan and explaining each step in detail. First, I carefully marked her deeper scars to decide on the best approach for each one. For some scars, I’ll be using my pinpoint CO2 laser, which precisely targets scar tissue, breaking it down while stimulating collagen to smooth and lift the skin. For others, where the scar depth and thickness make resurfacing alone insufficient, I’ll be performing excision to remove the scar entirely and create a more even surface. Next, I’ll perform subcision, a common technique I use to release the fibrous bands pulling the skin down and creating indented scars. Subcision allows the skin to lift naturally while stimulating collagen production for long-term improvement. Ready to book a consultation? Replying to @TheGreatColorWar Thank you so much for your comment and feedback! I want to emphasize that we never promise to achieve “perfect skin.” Perfect skin is subjective, and in reality, nobody has truly flawless skin. My goal is always to deliver a significant amount of improvement, but I believe it’s important to set realistic expectations. Promising “perfect skin” can create a false narrative and potentially harm someone’s confidence, which is something I take very seriously. These videos are meant to be more of a vlog-style glimpse into the day-to-day treatments we perform at our clinic. Acne scar treatments take time, and results typically develop over 6 to 18 months, as they depend heavily on the body’s natural collagen response. Many of our patients travel from out of state and don’t return to the office for in-person follow-ups, so we often rely on virtual check-ins to monitor progress. This makes it challenging to capture consistent before-and-after photos that fully showcase their results, which we know is important to see. Any results we do capture from patients who come in for follow-ups are posted promptly across our platforms—it may just take some scrolling to find them, which we understand can be frustrating. That said, we have a ton of before-and-after transformations featured here on our page, as well as on our website and Instagram. If you’re looking for specific examples, feel free to let us know, and someone from our team would be happy to send related results directly to your email for easier viewing. It’s easy to confuse acne scars with enlarged pores, as both can create uneven skin texture. However, they’re very different conditions, and their treatments are tailored accordingly: Acne Scars: These result from damage to deeper layers of skin caused by inflammation during active acne. Scars can appear as deep pits (ice pick scars), depressions (boxcar scars), or rolling textures. Effective treatments may include TCA Cross, subcision, laser resurfacing, or collagen-stimulating fillers. Enlarged Pores: These occur when oil glands produce excess sebum, causing the pores to stretch. Treatments for pores often focus on regulating oil production and refining skin texture with options like chemical peels, microneedling, or laser treatments designed for pore size reduction. During your consultation, we’ll take the time to analyze your skin and determine whether you’re dealing with scars, pores, or a combination of both. From there, we’ll create a personalized treatment plan to address your concerns Got questions about this process, your skincare or treatment options? When it comes to diagnosing facial fat loss, precision is key. That’s why I use angled lighting and evaluate my patients under different facial expressions to get a complete understanding of their volume loss. This method reveals subtle shadows and highlights that standard lighting can’t, giving me the most accurate picture of what’s happening beneath the skin. If your provider isn’t using angled or varied lighting during your consultation, it should raise some concern. Proper diagnosis is the foundation for effective treatment, and without it, addressing facial fat loss can miss the mark entirely. For this patient, the diagnostic process confirmed significant fat loss, which has left hollow areas and impacted the overall structure of their face. To restore their volume, I’ll be using Bellafill, a long-lasting filler made from a collagen base with microspheres that stimulate natural collagen production over time. By strategically placing Bellafill in areas of volume loss, we’ll recreate natural contours and restore balance, resulting in a refreshed and youthful appearance that lasts for years. The ablative CO2 laser is one of the most effective tools for deep, stubborn acne scars that won’t respond to surface-level treatments. This laser works by creating controlled micro-injuries in the skin’s surface, which breaks down damaged scar tissue while triggering your body’s natural healing response. Here’s how it works: 1. The laser’s energy vaporizes the outer layers of skin, removing damaged cells and creating tiny microchannels in the deeper layers. 2. These channels stimulate collagen production, which helps the skin rebuild itself from the inside out. 3. As the skin heals, new collagen fills in the indentations left by scars, resulting in smoother, more even skin texture. The ablative CO2 laser is particularly helpful for boxcar scars, rolling scars, and thick scar edges that need to be broken down at a deeper level. While it requires some downtime for healing, the long-term benefits of collagen remodeling make it a powerful treatment option for acne scars. Combining the CO2 laser with treatments like subcision, TCA Cross, or collagen-stimulating fillers can further enhance results, creating a comprehensive approach to addressing acne scars at every layer of the skin. This patient is back for the second phase of her acne scar treatment, and it’s exciting to see how much progress her skin has made since her first session! In her initial treatment, we focused on subcision, TCA Cross, and laser resurfacing to release and break down her deep scars. Now, her body’s natural collagen response has been activated, and we can see how the skin is starting to lift and smooth on its own. In this phase, I’ll be using my pinpoint CO2 laser to further break down the remaining thick edges of her scars. This laser allows me to target the scar tissue with precision, removing damaged skin at a microscopic level while leaving the surrounding skin untouched. This process triggers additional collagen production, which is essential for continued improvement. Treating acne scars is a gradual process that requires patience, as collagen builds over time. What is Sculptra and Why Do I Use It for Acne Scars? Sculptra is a collagen biostimulator, meaning it works by stimulating your body’s natural collagen production to restore volume and improve skin texture over time. Unlike traditional fillers that provide instant but temporary volume, Sculptra offers long-term results by helping your skin rebuild itself from within. Here’s how it works: Sculptra is made from poly-L-lactic acid (PLLA), a biodegradable substance that, once injected, triggers the body’s natural healing process to produce collagen. This collagen rebuilds the skin’s structure and adds volume where it’s been lost due to aging or scarring. For patients with acne scars, Sculptra is a game-changer. Acne scars often involve deep volume loss, especially in areas like the temples, cheeks, and jawline. Using Sculptra allows me to restore that lost volume while also smoothing the surface of the skin. The best part? Sculptra continues to work for months after treatment, producing gradual, natural improvements that last for up to 2 years or more. We are still accepting applications to be a Facial Balancing Model. Comment “Collagen” to get started. Spots are limited, and all treatments must be completed before April 1st, 2025, so don’t wait to apply! For this patient’s deep acne scars, we’re taking a comprehensive, multi-step approach using pinpoint CO2 laser, subcision, excision, and Erbium laser resurfacing to tackle the scars from every angle. Subcision is a crucial first step to release the fibrous bands beneath the skin that pull down and create indented scars. Without subcision, those scars remain tethered, which is why it’s necessary to free the skin before we move on to resurfacing techniques. In cases where scars are too thick or deep to improve with lasers alone, I perform excision to carefully remove the scar tissue entirely. This method allows us to achieve a smoother surface by removing scars that won’t respond to other treatments. Once the scars have been released and excised where necessary, I use a pinpoint CO2 laser to break down remaining scar tissue with precision, followed by the Erbium laser to smooth and refine the skin texture. The combination of these lasers targets both the surface and deeper layers of the skin to stimulate collagen production and promote long-term skin remodeling. This customized approach addresses the scars at every level—releasing, removing, and resurfacing to achieve smoother, healthier skin. Ready to book a consultation? This method allows us to achieve a smoother surface by removing scars that won’t respond to other treatments. Once the scars have been released and excised where necessary, I use a pinpoint CO2 laser to break down remaining scar tissue with precision, followed by the Erbium laser to smooth and refine the skin texture. The combination of these lasers targets both the surface and deeper layers of the skin to stimulate collagen production and promote long-term skin remodeling. This customized approach addresses the scars at every level—releasing, removing, and resurfacing to achieve smoother, healthier skin. Ready to book a consultation? When treating Asian skin types with ablative CO2 lasers, it’s critical to approach their treatment plan caution and precision. Asian skin, which typically falls into Fitzpatrick skin types 3-5, is more prone to post-inflammatory hyperpigmentation (PIH) and other complications if overheated or overtreated. The ablative CO2 laser works by creating controlled micro-injuries in the skin to vaporize damaged tissue and stimulate collagen production. While this technique is highly effective for acne scars, deep wrinkles, and skin resurfacing, it also generates heat. In skin types with more melanin, excessive heat can trigger unwanted pigmentation or inflammation. To safely treat my patients with Asian skin, I often choose to go fully ablative only in very specific areas where deep scars or thicker tissue require more aggressive treatment. In other regions, I adjust my settings to a fractional mode, which allows for targeted resurfacing without exposing the skin to unnecessary heat. This combination ensures that I can achieve significant results while minimizing risks. Got questions about this process, your skincare or treatment options? I’m so excited to get back to my scheduled lives starting tomorrow at 9 PM PST where I provide my real, unsponsored advice. I’ve really missed connecting with all of you and diving into your skincare questions in real-time. In this clip from a previous live, I’m breaking down the difference between Bellafill and other types of fillers. Bellafill is one of my favorite treatments because it’s not just about instant volume—it’s a collagen-stimulating filler that provides long-lasting results. Unlike hyaluronic acid fillers, which typically last anywhere from 6 to 18 months, Bellafill continues to work long after the initial treatment by encouraging your body to naturally rebuild collagen. The results can last up to 5 years, making it a great option for those looking for more long-term improvements. I also talked about other collagen-stimulating fillers like Radiesse and Sculptra, which also promote long-term collagen production, but each works a bit differently depending on your skin’s needs and treatment goals. This patient’s acne scar journey shows what a tailored, combination approach can achieve. By using multiple treatments targeting different layers of the skin, we were able to smooth and refine their skin texture significantly. Here’s a breakdown of what we did: TCA Cross: Precisely applied to target deep ice pick scars, stimulating collagen production to fill and lift the scar tissue. Subcision: Released the tethered scars by breaking up the fibrous bands beneath the skin, allowing it to lift naturally and trigger collagen remodeling. Pinpoint CO2 Laser: Targeted thick, uneven scar edges to resurface the skin and smooth out the texture by vaporizing damaged layers. Collagen-Stimulating Fillers: Restored volume in areas where scarring caused significant fat loss, ensuring a more balanced look. Erbium Laser: A gentler resurfacing treatment used to refine the outermost layers of skin and improve overall tone and smoothness. This combination approach addresses scars at every level: from surface texture to deep volume loss, ensuring the most natural and effective results. Youtube (@DrNadirQazi), Instagram and Twitch. Let’s talk about subcision—one of the most effective treatments for deep, tethered acne scars, but also one of the most technically challenging. Subcision involves using a blunt cannula to release the fibrous bands that pull down the skin, creating indented scars. These bands form during the acne healing process and can remain stuck for years, making scars appear deeper and more noticeable. It’s important to understand that subcision isn’t something to be taken lightly. It needs to be performed at the correct layer of the skin, typically in the subcutaneous plane, to avoid complications and ensure the best results. If subcision is done too shallow, it won’t fully release the scar tissue. If it’s done too deep, it can affect other facial structures, which is why going to an expert is critical. As you can see in this video, I take a meticulous and gentle approach while applying just the right amount of pressure to safely release scars that have been tethered for many years. In some cases, after subcision, I use fillers like a blended form of Restylane to restore lost volume in areas affected by scarring. The combination of subcision and volume restoration helps to lift the skin naturally and create smoother, more balanced results. Subcision is a key step in treating deep acne scars, but it requires skill, precision, and a deep understanding of facial anatomy to achieve natural-looking, long-term results. In this phase of treatment, I’m finishing the second half of this patient’s acne scar journey with a combination of lasers and Sculptra to tackle the remaining scar tissue and volume loss. I started with a pinpoint CO2 laser combined with a second CO2 laser to precisely break down the thick edges of the deeper scars. The lasers work by creating controlled micro-injuries, which trigger the body’s natural healing process and stimulate collagen production. This helps to smooth out the skin’s texture and improve the appearance of deep, stubborn scars. I also used Sculptra, a collagen biostimulator, to address the lost volume caused by acne scarring. Unlike traditional fillers, Sculptra doesn’t just add instant volume—it stimulates your body’s own collagen production for long-term improvement. I inject it deep into the skin, often at the bone level, to help restore natural facial structure and create a more balanced appearance. Post-treatment care is just as important as the procedure itself. For the best results, I recommend patients wear silicone tape over the treated areas to protect the skin as it heals. And remember, acne scar improvement takes time. It can take 6 months or more for your body’s natural healing response to fully kick in, and new skin development will continue to show over time.
