More general info about heart murmurs for you all!  After my last post that addressed the issue of “just monitoring” murmurs I got some feedback from other vets that sometimes clients have been offered diagnostics but declined them and that for many pet owners the cost to work up a murmur is restrictive. I agree! These are all great and true points.  Thus the unique complexity of veterinary medicine.  There are of course some situations where veterinarians offer all of the options and pet owners decline diagnostics for one reason or another.  I do feel strongly that we as veterinarians can always improve our communication skills and the way we approach these conversations.  I find that sometimes I can overwhelm clients with a barrage of expensive tests or make them feel we are overreacting to a new finding, especially if the patient is otherwise normal. These are often found during a routine annual exam so it can feel shocking to owners.  I find that a step by step approach to working up a murmur is often a really great plan.  Sometimes even giving a client a couple of weeks or more to digest the murmur news is a good idea.  They can come back for another visit where we listen to the heart again (love a complimentary exam for this), confirm the murmur is still present and start with just X-rays and/or proBNP testing if so.  Many pet owners can’t afford a complete cardiac work up which is when spectrum of care becomes really important.  Prioritizing the most helpful or affordable diagnostics can be a great start.  Working on a timeline to allow clients to save for diagnostics is also a great option for some.  In some situations where finances are a concern it is completely reasonable to start some heart medications without imaging!  What’s most important is that pet owners understand what the murmur can mean, what their options are to investigate further and what symptoms to look for that would indicate an issue in the future. We should always give our clients their options and then help them make the best plan based on several factors including finances  #vetmed #veterinary #heartmurmur
