As soon as you see it within the lumen, lower your angle of insertion, slide your probe away (proximal on the patient) a few millimeters, then advance your needle until you see it on the screen again, then slide the probe away, then advance again, all while keeping the bevel centered within the lumen of the vein. . This allows you to make sure there’s adequate catheter within the lumen, while visually confirming the needle has not entered the opposite wall. When drawing blood, occasionally the vacuum will suck the vein wall against the bevel of the needle, abruptly stopping blood flow, or causing a sort of “vibration”. . If this happens, try pointing your needle downward or simply pressing straight down with your needle to release it from the vein wall. This should restore blood flow. . If blood flow is still not restored, make sure your hands aren’t obstructing flow from distal veins, then try a new tube ( as the current tube may not have had enough vacuum). . The DOUBLE TOURNIQUET! . I get more messages about success with this technique than anything else. Give it a shot! . The purpose of a tourniquet when starting IVs is to limit venous return without affecting arterial flow (in order to dilate the veins by filling them with blood). . Most of the time a single tourniquet will do the job just fine, but sometimes you need to compress more tissue in order to restrict venous return. This is true of patients with excessive edema, obesity, or loose skin (though my volunteer here has none of these issues). . Using two tourniquets one on top of the other will create more pressure, and won’t fold over on themselves, preventing pinching of loose or delicate skin. . 🦵 Deep Vein Thrombosis (DVT) A deep vein thrombosis, or DVT, is a blood clot that forms in a deep vein in the body. 🩸 How it Forms:
1️⃣ Underlying Cause
From Virchow’s Triad (hypercoagulability, venous stasis, vessel damage)
 2️⃣ Platelets Collect at Valve
Clotting factors activate, producing fibrin → forms a mesh around WBC, RBC, PLT → clot forms and grows
 3️⃣ Clot Can Break Off
➡️ Forms an embolus, which may travel to the lungs and cause a Pulmonary Embolism (PE) 💥 Most Common Sites:
➡️ Calves: Peroneal Vein, Posterior Tibial Vein
➡️ Thighs: Popliteal Vein, Superficial Femoral Vein ⚠️ Risk Factors: Virchow’s Triad
🔴 Hypercoagulability: Increased tendency to clot
➡️ Causes: Sepsis, Dehydration, HIT, Oral Contraceptives, Genetic Factors
 🔴 Venous Stasis: Slowed or stagnant blood flow
➡️ Causes: Immobilization, Sedentary Lifestyle, Surgery, PVD, Pregnancy
 🔴 Vessel Damage: Damage to blood vessel walls
