This is a simple one, but still a question I get a LOT. Why can’t I thread the catheter after I see flash? . One of the most common mistakes I see is when someone gets flash, they immediately try to thread the catheter. This is going to cause problems because the moment you see flash, the catheter is not yet in the lumen of the vein, it’s still in the wall. . You’re seeing flash because the BEVEL is inside the vein, but if you look closely at your IV needle you’ll notice the catheter actually sits just behind the bevel. If you attempt to thread the catheter now, it will pull on the wall of the vein and often blow it if enough force is exerted. . When you get flash, pause, lower the angle of your needle, advance another 2-6mm (less with smaller gauges, more with larger ones), THEN thread your catheter. This one little tip will increase your success rate dramatically. . The idea is that when you reduce the angle and advance further, you’re inserting the needle up the lumen of the vein. This is even more important with larger gauges, and inserting at a slightly shallower angle may help as well (it should help prevent the tip of the needle from puncturing the other side of the vein). . Also, entering the skin swiftly can reduce pain. When palpating try to estimate how deep the vein is and insert your needle to that depth in one swift, smooth motion. .
