As soon as you see it within the lumen, lower your angle of insertion, slide your probe away (proximal on the patient) a few millimeters, then advance your needle until you see it on the screen again, then slide the probe away, then advance again, all while keeping the bevel centered within the lumen of the vein. . This allows you to make sure there’s adequate catheter within the lumen, while visually confirming the needle has not entered the opposite wall. When drawing blood, occasionally the vacuum will suck the vein wall against the bevel of the needle, abruptly stopping blood flow, or causing a sort of “vibration”. . If this happens, try pointing your needle downward or simply pressing straight down with your needle to release it from the vein wall. This should restore blood flow. . If blood flow is still not restored, make sure your hands aren’t obstructing flow from distal veins, then try a new tube ( as the current tube may not have had enough vacuum). . The DOUBLE TOURNIQUET! . I get more messages about success with this technique than anything else. Give it a shot! . The purpose of a tourniquet when starting IVs is to limit venous return without affecting arterial flow (in order to dilate the veins by filling them with blood). . Most of the time a single tourniquet will do the job just fine, but sometimes you need to compress more tissue in order to restrict venous return. This is true of patients with excessive edema, obesity, or loose skin (though my volunteer here has none of these issues). . Using two tourniquets one on top of the other will create more pressure, and won’t fold over on themselves, preventing pinching of loose or delicate skin. . Having a capable assistant is one of the most important aspects of pediatric venipuncture. They not only help hold the limb still, they can also help open supplies, and generally act as a second hand (since you’ll likely be holding the baby’s limb still with your other hand). . In this patient population it’s important to start IVs as low as possible, then work your way up if the IV loses patency. . IV removal isn’t complicated, but I see it done wrong way too often. If done incorrectly it hurts (ask me how I know 😉), and can cause further damage to the vein. . When discontinuing an IV, first remove all tape and dressings. If the patient has fragile skin or a statlock was used, alcohol will help prevent skin tears. Rub the edges with a stack of alcohol pads, then continue rubbing under the dressings as you remove them. . When all the tape/dressings are removed from the skin, fold a stack of gauze twice and hold it over the insertion site. DO NOT PUT PRESSURE DOWN UNTIL THE CATHETER IS COMPLETELY REMOVED. . Pull the catheter out slowly (faster is not less painful), and as soon as it exits the skin, put pressure down with your gauze. . Inspect the catheter to ensure that it is intact. If it isn’t, or you suspect that a piece of catheter was left in the patient, contact the physician. . Place a piece of tape crosswise over the gauze, stretching each side slightly before securing it to the skin. Here's another example of a tip I give a lot. The "Flash-pause-lower-advance-thread" technique. When you first get flash, the bevel of the needle is in the lumen of the vein. If you thread the catheter from here, you'll likely blow the vein as the end of the catheter may still be in the wall of the vein (it sits BEHIND the bevel of the needle). When you see flash, pause, lower your angle of insertion, advance another 2-6mm depending on gauge, THEN thread the catheter. This will make absolutely sure the catheter is within the lumen of the vein before threading and limit your chances of blowing it. . This is a simple one, but still a question I get a LOT. Why can’t I thread the catheter after I see flash? . One of the most common mistakes I see is when someone gets flash, they immediately try to thread the catheter. This is going to cause problems because the moment you see flash, the catheter is not yet in the lumen of the vein, it’s still in the wall. . You’re seeing flash because the BEVEL is inside the vein, but if you look closely at your IV needle you’ll notice the catheter actually sits just behind the bevel. If you attempt to thread the catheter now, it will pull on the wall of the vein and often blow it if enough force is exerted. . When you get flash, pause, lower the angle of your needle, advance another 2-6mm (less with smaller gauges, more with larger ones), THEN thread your catheter. This one little tip will increase your success rate dramatically. . The idea is that when you reduce the angle and advance further, you’re inserting the needle up the lumen of the vein. This is even more important with larger gauges, and inserting at a slightly shallower angle may help as well (it should help prevent the tip of the needle from puncturing the other side of the vein). . Also, entering the skin swiftly can reduce pain. When palpating try to estimate how deep the vein is and insert your needle to that depth in one swift, smooth motion. .
