Why is this an emergency? The sudden change in pupil size can signal something seriously wrong. In this case, it turned out to be a brain aneurysm. Asymmetric pupils could be a consequence of increased pressure in the brain - stroke, brain tumor, a bleed, or an aneurysm (which is when a blood vessel walls get thin so the vessel expands and pushes on the nerves that control the pupil muscles) could all be causes. If you have always had unequal pupils, then probably you don’t need to be worried. 10% -20% of the time there’s no reason behind the asymmetry and we call it physiologic anisocoria. But if you have a severe headaches, vision change, nausea, vomiting, pain, droopy eyelid, eye muscles that don’t move, it could mean something more serious is occurring and you need to get checked right away.
