The rigid diets and crazy workouts DON’T WORK menopausal and perimenopausal  women. But you probably already know that. Excessive cardio DOESN’T work, but you probably know that too. We know from the ELITE, EPAT and DOPS studies that 1- 2 mg per day of oral estradial were cardioprotective.  1 mg oral estradial gets you to 65 pg/ml of serum estradial. ✅ In the KEEPS study, 50 ug estradial patches did NOT reduce CV disease. This corresponds to a serum E2 level of 45-55 pg/ml.❎ Bones need less estrogen but higher doses protect better than lower doses in studies. 🦴 Oral Estradial has stronger evidence than transdermal estradial for CV protection. ♥️ My goal for serum estradial is 65 - 125 pg/ml for most women in HRT in menopause. Why don't we check blood hormones in women taking HRT? 🤔 One reason: Doctors don't think HRT is a legitimate disease treatment or preventative strategy.  They think HRT in menopause is just fluff.  If something is just for symptom relief then you only need to look at symptoms to know if it's working. ✅ However, if a hormone is meant to treat or prevent a disease, guidelines are established around optimal blood hormone levels. ❎ Such guidelines have never been established for HRT because few doctors view it as a serious disease prevention tool.  So, while I have goals for estradial and progesterone in menopausal women on HRT, many doctors don't. And, no guidelines have been established.
