V@ginal progesterone works great for women who need uterine protection but can't take oral progesterone due to side effects like drowsiness and depression. Whether a hormone acts locally (only where you place it) or systemically (on your whole body) depends on the concentration and dose. βœ… Higher doses of hormone can act systemically, even if given vaginally.  Lower doses, like what we use with low dose vaginal estrogen for GSM (or low dose vaginal estrogen for the face) act primarily where they're applied. 😁 But, even a low dose vaginal estradiol (ex 0.01% = 0.1mg/gm) can act systemically if you apply enough.  For example, if you apply 3 g to your vagina and another 5 g to your face, neck and chest, you're getting 0.8 mg of estradial, which is similar to the dose we'd use in a 1 mg/gm estradial (systemic) cream.  ⚠️ Yes, it's complicated. And, fyi... To make things even more complicated, some symptoms, like depression, anxiety, irregular periods and bloating, can be due to high estrogen OR high progesterone! 🀬 If you're not sure, I recommend talking to your doctor. Sometimes blood tests help. Sometimes they don't (ex in perimenopause).  Charting symptoms can be useful.πŸ“‰ Do you feel more high E, high P or low in both? ❓ P.S. The above are "classic" symptoms but that doesn't mean that everyone with these symptoms has high E or high P. It's been a rough day. 😫😫 Under the brand name, on your label, look for "estradial" or "17b estradial" βœ… Under the brand name, look for "progesterone" or "micronized progesterone". βœ… If you can't find the "generic" name, Google the brand name and find the drug info page.  If you see any other words or variations, it is likely that what you're taking is not bioidentical. ❎ That doesn't necessarily mean it's a problem. Some people do better with progestins than progesterone.  But, some progestins, like the now-infamous medroxyprogesterone acetate, can be problematic. Alzheimer’s affects women at a much greater rate than it does men and Dr. Lisa Mosconi @dr_mosconi wrote an amazing book about this and is currently starting a $50 million research project about this issue. The main point is that the decline of estrogen is linked to the onset of dementia/Alzheimer’s.
